As a client, you have the right to:

Restrict Disclosures: Request limitations on how we use or share your PHI
Confidential Communications: Request communications through alternative means or locations
Receive an Accounting of Disclosures: Learn how your PHI has been shared (excluding treatment, payment, or operations)
File a Complaint: If you believe your privacy rights have been violated, you can file a complaint with us or the U.S. Department of Health and Human Services
We will never deny you care or retaliate in any way for filing a complaint.
4. Safeguarding Your Information
We use a combination of administrative, physical, and technical safeguards to protect your information. All staff members are trained in HIPAA compliance and confidentiality protocols.

We may update this Notice at any time. When we make changes, we will post the revised version on our website and provide it upon request.
